Woman given unneeded mastectomy after test results mix-up

A woman recently received an unnecessary mastectomy after test results were somehow switched in two Capital Health patients’ charts. The health authority announced a second, similar incident also related to pathology tests. This one led to an unnecessary diagnostic biopsy for one patient while another patient needing follow-up care at that time didn’t receive it. Halifax Chronicle-Herald story.
